You might have some luck with Stationary Construction Kit from Standalone.
It lets you create additional notes with any kind of widgets on it. There
was also another application that would let you do something similar, except
that you had to load the forms into the application itself, it was not
creating Notes stationary. I think it was named "PowerForms" or something
like that. However, both of them lacked the analysis you might be looking
for.
